Setting Up Forms in Zanda: Your Options and Getting Help

Understand how to get your forms into Zanda, what the Template Library and Form Designer can do, when to convert a note template, and how to find additional help.

Do I have to build my forms from scratch?

If your practice uses existing forms in Word, PDF, or paper format, it’s important to know that these cannot be imported directly into Zanda as interactive forms.

Zanda forms are built natively within the platform.

You still have several flexible options depending on your situation:

Option 1: Start with the Template Library

The fastest way to get started is to check the Template Library.

Zanda offers a growing collection of ready-made templates for common use cases, including intake forms, consent forms, and outcome measures.

To import a template from the library

  1. Go to Tools > Form Designer
  2. Click Template Library (top right)
  3. Browse or search for a template
  4. Click a template to preview
  5. Scroll down and click Save to My Account
  6. Click the gear icon and select Edit to customise

Important: Templates are provided as a starting point. You are responsible for adapting them to meet your legal and professional requirements.

Option 2: Build from scratch with the Form Designer

If you need a fully custom form, you can create one using the Form Designer.

To create a new form template

  1. Go to Tools > Form Designer
  2. Click + New Form Template
  3. Enter a name and select Admin or Clinical
  4. Add sections and fields
  5. Click Save

Available field types include

  • Single-line and multi-line text
  • Multiple choice (radio buttons, checkboxes, dropdowns)
  • Yes/No toggle
  • Signature
  • Draw/Annotation (e.g., body diagrams)
  • Client file upload
  • Linked Profile Fields (auto-update client profiles)
  • Information blocks (instructions, headings, terms)

You can preview how your form looks to clients by selecting View > Data Entry.

Pro tip: You can export a form template as a .txt file and import it into another Zanda account via: Tools > Form Designer > Import From File

 


Option 3: Upload a PDF as a static client file

If you only need to store a completed document (not collect new data), you can upload it as a file.

To upload a PDF

  1. Open the client’s profile
  2. Go to the Files section
  3. Upload the document

Important: These files are static. Clients cannot complete or sign them inside Zanda.


Converting an existing Zanda note template to a form

You can convert an existing Zanda note template into a client-facing form.

This works well for things like pre-session questionnaires that are currently used internally.

Important:

  • This only applies to templates already created inside Zanda
  • External files (Word, PDF, Google Forms, etc.) cannot be converted
